Group Name: Black Creek Community Health Centre – Youth Programs

Description: BCCHC is a non-profit community based organization that provides health care services in a holistic manner and works with people to create safe and healthy communities. We are funded by the Central Local Health Integrated Network (Central LHIN) and the Ministry of Health and Long Term Care.

These programs support youth (14-29 years of age) in their transition into adulthood. They include a variety of intervention initiatives to promote healthy lifestyle choices such as sexual health, substance use, education, employment readiness and social skills development. *** Black Creek Community Health Centre provides services to community residents who live in our service area or 20, 30 or 40 Falstaff Ave. We do not operate as a walk-in clinic. Keele – East, Humber River – West, Steeles – North, Wilson – South.
