Group Name: Toronto Public Health – Healthy Families & Healthy Living Intake

Description: To provide health education and community support for mothers-to-be and new parents. To provide support for parents who have experienced a stillbirth or the loss of a newborn.

Customer Service Representatives of Toronto Health Connection link callers to programs and services of the City of Toronto. Public Health Nurses of Healthy Families and Healthy Living Intake provide telephone health information and counselling and referrals to the Healthy Babies Healthy Children Program. Intake Registered Dieticians provide nutrition counselling and referrals to the Healthiest Babies Possible Program.

District Public Health Nurses provide parent education sessions at a variety of community locations. Public Health Nurses and Nutritionists collaborate on prenatal programs in the community.
