Group Name: Chronic Disease Education & Support Clinic – Smoking Cessation

Description: The Chronic Disease Education & Support Clinic is located on the ground floor of the Rouge Valley Health System building. Rouge Valley offers education programs to support patients living with many different chronic disease conditions. These programs will help you learn to manage your condition and prevent it from worsening. This is an OHIP funded clinic that provides education, nutrition and exercises for diseases and conditions such as arthritis, COPD, chronic pain, osteoporosis, hypertension, asthma and smoking cessation.

Although quitting smoking can be hard, it is possible. There are many ways and people out there who can help you quit. Our program is five weeks long, with two sessions per week. These sessions consist of group education and interactive learning. Classes are offered in a relaxed and inviting atmosphere, so that you can feel comfortable. Our program is led by a registered practical nurse. You and your family are also important members of the program team. We encourage you to bring one support person with you for classes. When requested, individuals can arrange to meet with a registered dietitian the following day at Rouge Valley.

When: Meetings are held on Tuesdays and Thursdays from 6-6:30 p.m.
