Description: The South Riverdale Community Health Centre (SRCHC) is a community-based organization that offers primary health care services and health promotion programs to a diverse community. We strive to respond to local issues and provide services in active partnership with our clients, patients, neighbours, community groups and professional organizations. SRCHC is funded by the Toronto Central Local Health Integration Network.
We offer an 8-session support program for grandparents caring for grandchildren under the age of six years. Activities include: group discussions, and learning though games and interactive fun. Healthy snacks, are provided to all participants. Client information and learning needs inform the choice of topics. Examples include: Active living for seniors, healthy child development, family relationship, navigating the healthcare system, and many more. A Nurse Practitioner, Community Dietitian, Health Promoter and Social Worker plan and facilitate these topics. Childcare service is available to participants who needs it. Participants who lack transport facilitation may be eligible to receive TTC tokens. Individual consultation and service referrals are also available. Newcomers to Canada are very welcome! ***Grandparents with grandchildren under six. Priority is given to those living within service boundaries. Appointments and registration are required for this program. Languages: English, Mandarin, Cantonese.
