Description: St. Clair West Services for Seniors is a non-profit charitable organization which provides valuable, innovative, and caring support services to older and/or disabled adults who wish to maintain their quality of life while living in their own homes. We provide a range of services, including Adult Day Services, Case Management, Home Help, Meals on Wheels, Respite Care, Supportive Housing and Transportation. We have offered our services in the community since 1973 and we endeavour to provide these services in a variety of languages by trained staff and volunteers. Our community development focus creates opportunities to empower individuals and groups and builds sustainable, healthy communities.

At our Adult Day Centre we provide supervised social, recreational and therapeutic programming for seniors and/or adults with disabilities who are unable to attend regular community activities because of physical and/or cognitive impairment. Our goal is to support and encourage the participants’ independence and optimal functioning and to ensure their dignity is maintained. We provide individualized program plans that reflect the participants’ interests, culture and functional abilities. Montessori-based activities and Snoezelen therapy are our specialties. Special areas of focus are for frail seniors and seniors with Alzheimer’s. ***Service area includes residents west of Spadina to Humber River and north of St. Clair Avenue West to Lawrence Avenue West.

Etobicoke Services for Seniors – Day Program

Description: Etobicoke Services for Seniors (ESS) is a not-for-profit agency committed to supporting seniors in their desire to remain in their own homes and community. Their agency’s programs, services and facilities have been designed to meet the unique needs of the elderly in our community. Individuals who access our services may be frail, isolated, living with memory-related disorders such as Alzheimer’s Disease, or recovering from a stroke.

Our Adult Day Service program provides support to physically frail and cognitively impaired seniors and adults. Participants are offered a secure environment to participate in mental and gentle physical exercises, social and recreational activities. Individual interests and hobbies are incorporated into the program, snacks and meals are tailored to meet dietary restrictions and transportation to and from the program is available.

Description: Luma Care aspires to enhance, engage, and empower the members of their diverse community by providing equitable, accessible and high quality services. They also achieve excellence and provide leadership in family-centric health and social services in the community. They provide resources such as Meals on Wheels, transportation, home support, Diners Club, Home at Last, and many more.

Branson Place is an Adult Day Program for individuals with Alzheimer’s Disease or related dementia. Clients participate in programs that reaffirm dignity and stimulate body and mind. In a safe, secure homelike environment, participants engage in a range of meaningful and appropriate social therapeutic programs. Branson Place provides caregivers peace of mind, knowing that their loved ones are being cared for. Caregivers can take this time for themselves to relax or complete their to-do lists.

WoodGreen Community Services – Adult Day Program

Group Name: WoodGreen Community Services – Adult Day Program

Description: Celebrating 75 years of success in 2012, WoodGreen Community Services is one of the largest social service agencies in Toronto. A founding United Way of Toronto member agency, WoodGreen has grown to span 32 locations and serves 37,000 people each year. WoodGreen helps people find safe, affordable housing, seniors live independently, internationally-trained professionals enter the job market, parents access childcare, children and youth access after-school programs, newcomers settle in to Canadian life, homeless and marginalized people get off the streets and youth find meaningful employment and training.

The Adult Day Program is a supervised social recreational program for English and Chinese speaking seniors who are frail or have dementia. We provide escorted transportation to and from the program for seniors living within our catchment area. Our program specializes in people who are living with the effects of a stroke and creates an opportunity to interact with peers in supportive stimulating environment. This program will serve as an alternative respite measure for caregivers in need of relief and support.

The Transitional Adult Day Program is for individuals living in the community who wish to participate in social programming and connect with peers, but require additional support throughout the day. This innovative new program will offer more assistance to physically frail seniors or those with mild cognitive impairments, while providing beneficial recreational programming and structured social engagement. Description: Yee Hong is a caring community where seniors and others enjoy the highest quality of life. We raise the standard in long-term care and effect a positive impact on the global health care system by seeking to be the model of excellence in culturally appropriate geriatric care. We take pride in being a provider of choice and an employer of choice.

The Adult Day Program aims to provide structured and supervised activities for post-stroke and dementia patients and frail seniors in order to prevent social isolation and encourage independent living in the community. To offer social, recreational, physical and educational activities in order to maintain the optimal level of functioning and delay institutionalization, and to provide respite, support and information services for caregivers. Program includes:

  • Educational, physical, recreational and social activities
  • Assistance with activities of daily living
  • Individual/group supportive counselling
  • Snacks and hot lunch
  • Foot care service
  • Caregiver support program

Referrals are welcome from medical professionals, social workers, family members, friends or clients themselves. Personal interview is required to determine eligibility. Application for the Mississauga Day Program should go through the Community Care Access Centres.

Description: Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area, and the York Region.

The Adult Day Program provides supervised activities for frail or socially isolated Chinese older adults. Our goal is to enable seniors to achieve and maintain their maximum level of functioning, to prevent premature and inappropriate institutionalization, and to provide respite and support for caregivers. Eligibility:

  • Seniors aged 55 or above and or those with physical disability
  • Seniors who experience functional limitations due to physical behavioural and cognitive impairments.
  • Seniors who experience social isolation with limited family and/or peer support
  • Seniors who are unable to participate independently in regular community program

Description: The Second Mile Club’s mission is to provide resources and services which contribute to the enhancement of the social, intellectual and physical well being of a multicultural community of seniors and disabled adults in the City of Toronto. We will achieve this vision in the in the community through these specific means:

  • Provide accessible, welcoming facilities that offer various activities to promote social and intellectual participation.
  • Provide a Community Support Program that offers the necessary assistance to seniors and disabled adults to enable them to live independently in their homes and to provide support for their caregivers.
  • Continuously seek to identify and address emerging needs of seniors and disabled adults and to enhance the variety of our activities and the quality of our services.
  • Seek out and develop partnerships and collaborations with other agencies and municipal and provincial governments to ensure that we continue to maintain the highest level of services to seniors and disabled adults.

This facility based support program is designed for the frail older adult, who requires specialized programs, small group activities, nutritious meals and transportation. The program provides for caregiver respite & support. Individuals who are 55 years of age and older and individuals with physical disabilities, who are over 18 years of age. ***Applicants will be assessed prior to admission to this program and should live in the area bounded by Bathurst Street, Don Valley Parkway, St. Clair Avenue West and Lake Ontario.

Better Living Health and Community Services – Adult Day Program

Description: Better Living Health and Community Services has been delivering community support services in the Don Mills area since 1976. Their mission is to support individuals in our community in maintaining their independence, enhancing their social wellbeing, and optimizing their overall health and wellness through the provision of a wide range of community support services and leisure activities. They are committed to our mission by helping adults and seniors, at all stages, experience a better quality of life.

The Adult Day Program provides a safe and stimulating environment for adults with disabilities and seniors 55+ who are frail, isolated, or managing the cognitive challenges of Alzheimer’s, Dementia, Parkinson’s or Stroke. ADP also plays a critical role in providing respite for Caregivers who are supporting their loved ones at home. The Adult Day Program is located within the Better Living Community Centre and is fully accessible for Clients using wheelchairs or other assistive devices. The program area features secure entry and exit points, a dining area with a full industrial kitchen, two large programming areas, and a staff office.

When: The Adult Day Program is available Monday to Friday from 9:00 am to 3:00 pm and select evenings and weekends. Clients may choose to attend for full or half days up to six days a week. Additional ‘flexible’ support hours are available to those who regularly attend the program. Subsidies are available for both half and full day.
