Description: The Davenport Perth Neighbourhood and Community Health Centre supports our community’s residents at every stage of their lives. Our Neighbourhood Centre is a non-profit, multi-service organization that works towards building supportive communities, while our community Health Centre provides primary care for clients using a multidisciplinary team of doctors, nurses, nurse-practitioners, counselor/therapists, dietitians and medical administration staff. DPNCHC provides both of these services to its community in one convenient location.

If you feel like you could use some help integrating into the neighbourhood, finding a job, or accessing resources, there’s a place for you at DPNCHC. Adult Services programs are the place to socialize, make friends and access community resources. Here, you can benefit from recreational activities, street outreach, harm reduction services and crisis intervention and support. Continue learning at any age by upgrading your reading, writing, math and basic computer skills. Or enjoy the hearty and nutritious meals served during our community dining program.

Hong Fook MHA – South Asian Mental Health Clinic

Group Name: Hong Fook Mental Health Association & Mount Sinai Hospital (Assertive Community Treatment Team)

Description: Hong Fook Mental Health Association was established in 1982 to address the mental health concerns in the East Asian communities. They currently serve these communities:
Cambodian, Chinese, Korean, and Vietnamese. Mount Sinai Hospital is a 446-bed acute care academic health sciences centre affiliated with the University of Toronto that is dedicated to delivering the best medicine and best patient experience. It is focused on excellence in patient and family-centred care, innovative education and leading-edge research.

The Association aims to help people with linguistic and cultural barriers to gain access to mental health services. They provide the following services to clients and their families:

  • Counseling
  • Case management
  • Assessment
  • Consultatio
  • Advocacy
  • Psycho-educational workshops and support groups
  • Special ESL (English as a Second Language) classes
  • Housing services

Community Parent Outreach Program

Group Name: West Neighbourhood House- Community Parent Outreach Program

Description: West Neighbourhood House will work in partnership with the community to promote personal and social change in order to achieve a safe, health and accepting society for all. To this end, we will work with our resources and the strengths of the community. West Neighbourhood House enables less advantaged individuals, families and groups in the community to gain greater control over their lives and within their community.

The central purpose of the Community Parent Outreach Program is to reduce the social isolation of parents from diverse cultures with children 0-6 years old an increase social independence. Languages spoken relating to this group are Tamil, Singhalese, English, Mandarin, Tibetan, and Vietnamese. Participation in the program will:

  • Enable you to build new friendships
  • Enhance your parenting skills
  • Help you adjust to Canadian culture
  • Provide opportunities for you to become active on issues you care about
  • Increase your understanding of diverse cultures
  • Provide accompaniments to community Services
  • Provide information about Parkdale community resources

Community Resource Connections of Toronto – Wellness Group

Group Name: Community Resource Connections of Toronto – Scarborough Wellness Group

Description: CRCT operates an active web site containing information about events, news and resources. The web site also has a searchable database of mental health services, supports and resources available in Toronto. We are working to provide a comprehensive source of information which meets the needs of consumer/survivors, family members and service providers.

This is a free activity for adults in the Scarborough area who are living with mental health issues. It is a wellness-based recovery program being developed by people recovering from mental health issues. Suggested activities include: Learning and sharing about recovery through guest speakers and videos; Yoga; Nutrition and fitness; Creative Art; Music appreciation; Self Advocacy; Out and About: walking group; Peer support; And much more and your suggestions are welcome.
