Group Name: Chronic Disease Education & Support Clinic – Osteoporosis

Description: The Chronic Disease Education & Support Clinic is located on the ground floor of the Rouge Valley Health System building. Rouge Valley offers education programs to support patients living with many different chronic disease conditions. These programs will help you learn to manage your condition and prevent it from worsening. This is an OHIP funded clinic that provides education, nutrition and exercises for diseases and conditions such as arthritis, COPD, chronic pain, osteoporosis, hypertension, asthma and smoking cessation.

The Osteoporosis meetings were created to help individuals cope with disease on a day to day basis. Individuals in attendance will be given take home work to complete and will have an opportunity to meet others who have osteoporosis. When requested, individuals can arrange to meet with a registered dietitian the following day at Rouge Valley.

When: Meetings are held on Tuesdays from 5 p.m. until 6 p.m. and Thursdays from 11 a.m. until 12 p.m.

Ajax-Pickering Osteoporosis Support

Group Name: Ajax-Pickering Osteoporosis Support

Description: To help individuals cope with their osteoporosis on a day to day basis. Guest speakers and other educational resources are provided at the meetings.

Where: St. Paul’s United Church 65 King’s Crescent | Ajax, ON L1S 2M4

When: Meetings are held on the 1st Wednesday of every month from 1:30-3:00 pm. Excluding the months of June, July, August and December. Membership is $10/year or $2 per attendance. Light refreshments are available for $1.

Group Name: Alzheimer Durham

Description: Alzheimer Society of Durham Region provides help for today by working with local and regional agencies to meet the needs of people with dementia. They also provide support and education programs for people with Alzheimer’s disease and related dementia, and their family and professional care partners.
