Description: St. Clair West Services for Seniors is a non-profit charitable organization which provides valuable, innovative, and caring support services to older and/or disabled adults who wish to maintain their quality of life while living in their own homes. We provide a range of services, including Adult Day Services, Case Management, Home Help, Meals on Wheels, Respite Care, Supportive Housing and Transportation. We have offered our services in the community since 1973 and we endeavour to provide these services in a variety of languages by trained staff and volunteers. Our community development focus creates opportunities to empower individuals and groups and builds sustainable, healthy communities.

At our Adult Day Centre we provide supervised social, recreational and therapeutic programming for seniors and/or adults with disabilities who are unable to attend regular community activities because of physical and/or cognitive impairment. Our goal is to support and encourage the participants’ independence and optimal functioning and to ensure their dignity is maintained. We provide individualized program plans that reflect the participants’ interests, culture and functional abilities. Montessori-based activities and Snoezelen therapy are our specialties. Special areas of focus are for frail seniors and seniors with Alzheimer’s. ***Service area includes residents west of Spadina to Humber River and north of St. Clair Avenue West to Lawrence Avenue West.

Description: The Anne Johnston Health Station (AJHS) is a not-for-profit community health centre providing a wide range of programs and services that promote the health and well-being of youth, seniors and people with physical disabilities. They are a fully accessible barrier free environment. All of our programs and services are confidential and free of charge and include primary health care, disease prevention, health promotion and community development. We believe that health is a resource for life and that many things – education, housing, gender, access to health services, our sense of being included or excluded from society, employment, etc. – impact on our health. Health isn’t just about not being sick, it is about being able to make choices and live into them. It’s about having a voice in our community. They are guided by our mission, vision and values. The Anne Johnston Health Station is governed by a Board of Directors elected from our membership.

SAGE (Seniors Acheiving Greater Esteem) promotes the health and well-being of frail and/or isolated seniors by encouraging social engagement and independence. Interested participants must register prior to attending – this is not a drop-in class.

WoodGreen Community Services – Friendship Club

Group Name: WoodGreen Community Services – Friendship Club

Description: Celebrating 75 years of success in 2012, WoodGreen Community Services is one of the largest social service agencies in Toronto. A founding United Way of Toronto member agency, WoodGreen has grown to span 32 locations and serves 37,000 people each year. WoodGreen helps people find safe, affordable housing, seniors live independently, internationally-trained professionals enter the job market, parents access childcare, children and youth access after-school programs, newcomers settle in to Canadian life, homeless and marginalized people get off the streets and youth find meaningful employment and training.

Friendship Club runs a range of activities that the group members decide on. Groups include women’s beading/jewellery making, guitar workshop (Tuesday and Thursday mornings 9:30 – 11:00), games, a meal, karaoke and arts and crafts. Please contact Gillian Edwards for the month’s schedule of activities. Program components provide social and recreational opportunities for community members who may feel isolated. This program is only for adults who feel isolated due to mental health, developmental, substance use, health or street involvement. The Friendship Club provides a respectful and fun environment where community members can connect with each other while enjoying themselves.

Description: Etobicoke Services for Seniors (ESS) is a not-for-profit agency committed to supporting seniors in their desire to remain in their own homes and community. Their agency’s programs, services and facilities have been designed to meet the unique needs of the elderly in our community. Individuals who access our services may be frail, isolated, living with memory-related disorders such as Alzheimer’s Disease, or recovering from a stroke.

Our Adult Day Service program provides support to physically frail and cognitively impaired seniors and adults. Participants are offered a secure environment to participate in mental and gentle physical exercises, social and recreational activities. Individual interests and hobbies are incorporated into the program, snacks and meals are tailored to meet dietary restrictions and transportation to and from the program is available.

Description: Silver Circle’s mission is to help seniors live independently for longer and for those who care for seniors to be able to provide care by having: respite, physical and emotional support, and the information to make informed choices.

The Adult Day Program is a supervised social recreational program for English and Chinese speaking seniors who are frail or have dementia. We provide escorted transportation to and from the program for seniors living within our catchment area. Our program specializes in people who are living with the effects of a stroke and creates an opportunity to interact with peers in supportive stimulating environment. This program will serve as an alternative respite measure for caregivers in need of relief and support.

Luma Care – Transitional Adult Day Program

Description: Luma Care aspires to enhance, engage, and empower the members of their diverse community by providing equitable, accessible and high quality services. They also achieve excellence and provide leadership in family-centric health and social services in the community. They provide resources such as Meals on Wheels, transportation, home support, Diners Club, Home at Last, and many more.

The Transitional Adult Day Program is for individuals living in the community who wish to participate in social programming and connect with peers, but require additional support throughout the day. This innovative new program will offer more assistance to physically frail seniors or those with mild cognitive impairments, while providing beneficial recreational programming and structured social engagement. Description: Yee Hong is a caring community where seniors and others enjoy the highest quality of life. We raise the standard in long-term care and effect a positive impact on the global health care system by seeking to be the model of excellence in culturally appropriate geriatric care. We take pride in being a provider of choice and an employer of choice.

The Adult Day Program aims to provide structured and supervised activities for post-stroke and dementia patients and frail seniors in order to prevent social isolation and encourage independent living in the community. To offer social, recreational, physical and educational activities in order to maintain the optimal level of functioning and delay institutionalization, and to provide respite, support and information services for caregivers. Program includes:

  • Educational, physical, recreational and social activities
  • Assistance with activities of daily living
  • Individual/group supportive counselling
  • Snacks and hot lunch
  • Foot care service
  • Caregiver support program

Referrals are welcome from medical professionals, social workers, family members, friends or clients themselves. Personal interview is required to determine eligibility. Application for the Mississauga Day Program should go through the Community Care Access Centres.
