Description: The Chronic Disease Education & Support Clinic is located on the ground floor of the Rouge Valley Health System building. Rouge Valley offers education programs to support patients living with many different chronic disease conditions. These programs will help you learn to manage your condition and prevent it from worsening. This is an OHIP funded clinic that provides education, nutrition and exercises for diseases and conditions such as arthritis, COPD, chronic pain, osteoporosis, hypertension, asthma and smoking cessation.

Our hypertension education program is delivered at Rouge Valley Ajax and Pickering hospital campus. The program has been designed for individuals who are at risk and would like to learn to be proactive about their health. You do not need a physician referral to register for the program. Our program is built on an education module from Hypertension Canada as well as the Heart and Stroke Foundation. Our goal is to help you and your family to develop the knowledge and skills to manage your hypertension on a day-to-day basis. This includes understanding how to adjust and maintain your behavior and lifestyle as needed. When requested, individuals can arrange to meet with a registered dietitian the following day at Rouge Valley. Pulmonary Hypertension Association of Canada – Toronto Chapter

Group Name: Pulmonary Hypertension Association of Canada – Toronto Chapter

Description: The Toronto Chapter was formed by a volunteer group of patients, families and caregivers in the Fall of 2008. Patients found each other through a pulmonary hypertension group on Facebook and got together at a local coffee shop. They shared and exchanged stories of how PH has affected their lives and how they deal with it. It was amazing to see how each person had a different story yet they could still relate to how each other felt. Strong bonds were immediately formed.

The Toronto Chapter is devoted to helping improve the lives of those living in the GTA who are affected by Pulmonary Hypertension. We meet on a regular basis to share our experiences and support each other. *** These are personal numbers so please do not call before 9 a.m. or after 8 p.m.

Where: Toronto, ON

When: The Support Group meets once a month (usually the first Saturday afternoon of the month). Their meetings are open to everyone, whether you are a patient, caregiver, family/friend or just someone who is interested to learn more about PH. Call for more information.
